The Colorado Department of Public Health and Environment (CDPHE) proposes the following strategies to prevent obesity and related chronic diseases in Colorado:
  • To convene a Steering Committee of internal and external partners to oversee the creation of a state plan to integrate nutrition and physical activity intervention through community-based strategies using policy-level change, environmental supports and the social marketing process.
    Conduct a pilot intervention to increase physical activity and encourage healthy eating habits. The pilot program will use a community-based prevention marketing approach and will represent two communities: one high-risk population (Hispanic/African American) and one community that is already implementing activities and strategies to reduce and prevent obesity.
    Use of the Department’s existing infrastructure of chronic disease programs with established coalitions to disseminate messages and implement strategies developed by the Steering Committee.

    Allergy specialists can provide you with complete allergy evaluations, testing, and injections by referral.     Cold Clinic This walk-in clinic located in the medical clinic is staffed by registered nurses and nurse practitioners to assist you in assessing your symptoms and determine if you need a throat culture or further medical care. Dining with Diabetes in Colorado: An Example of a Successful Collaboration
Background/Situation and Target Audience

Diabetes impacts approximately 16 million Americans each year. Diabetes rates are increasing so rapidly that the CDC now considers it an epidemic. In Colorado, the number of cases in the state has risen by an estimated 26% since 1990. Additionally, Colorado is one of the fastest growing states in the U.S. with a highly diverse population. County Extension agents, who typically field general nutrition questions from consumers, have reported receiving more questions in the past few years related to diabetes and basic meal planning. This rise in diabetes cases raises many issues in Colorado communities. Many small and rural towns do not have registered dietitians or diabetes educators to receive referrals. Even in larger towns and cities with registered dietitians and/or diabetes educators, it is difficult for health professionals to provide both initial comprehensive education and on-going education to patients with diabetes, given the increase in diagnosed cases, the severity of diabetes, and the current workload.
To address the demands and needs of individuals with diabetes, Colorado Diabetes Control Program (DCP) and Colorado State University Cooperative Extension agreed to form a partnership. The main goal of this collaboration was to develop an educational approach that would supplement information provided by registered dietitians and qualified health care providers, while providing practical hands-on meal-planning experience. The program does not replace diabetes education delivered by qualified health professionals.

Colorado Connection for Healthy Kids (CCHK) is a coalition of Colorado organizations working together to promote the health and well-being of all children, youth and families through coordinated school health programs.
Housed within the Colorado Foundation for Families and Children, under its Center for Management and Support, CCHK is a state-level organization supporting local school health efforts by affecting policy at the legislature through a fast e-mail mobilization system, disseminating information across the state, conducting a statewide survey on school health for the State Board of Education, and becoming the lead agency for Colorado on the State Collaborative on Assessment and Student Standards (SCASS) project with the Centers for Disease Control and Prevention. Colorado is a model for the country in that it is the ONLY state in the nation to raise the substantial membership fee for SCASS through a state collaborative effort.
