previous page Some files are in Adobe Acrobat The Child Care Licensing Division of the Texas Department of Protective and Regulatory Services (PRS) provides the following forms for use by:
  • child care providers day care centers, registered family homes, group day homes, child-placing agencies, 24-hour care facilities, emergency shelters, foster homes, foster group homes, and agency foster group homes.
This information is required by PRS and simplifies form completion for providers. The form reduces errors in the information that must be presented to request these background checks. After completion, send the form to the address listed on the form. All of the other forms are tools for providers to use, if they wish, to simplify recording keeping at the facility and remain at the facility. The records are periodically reviewed by licensing staff at each facility.

This workshop is for parents and child care providers and focuses on the use of television to motivate, teach, illustrate a concept in another way, and accommodate different learning styles. MEDIA VIOLENCE -
This workshop explores the use of violence in today’s media and how parents can protect their children from it. READY TO LEARN ABOUT CONFLICT-
This workshop is designed to teach how to solve conflicts between children, or between adults and children. The workshop promotes a positive approach, using short video clips from various PBS programs. CULTURAL DIVERSITY:

  • This initiative is already locating the best of what exists and developing new materials that directly support the education and training of those involved in providing child care - and especially family child care - across America. Print, video, software, and ways to efficiently disseminate them are all critical aspects our efforts. In addition, this Initiative will seek to bring together the best thinking and policy alternatives and then disseminate this important information to policy-makers nation-wide from local zoning officials and small-town Boards of Supervisors to congressional staffs - utilizing a variety of new and emerging distribution technologies and providing an important national policy forum.

Idaho Child Care Program Information for Parents About Direct Payments to Child Care Providers Information in Spanish
The Idaho Department of Health and Welfare is making some changes in the Idaho Child Care Program (ICCP). Beginning in May 2002, ICCP payments will be made directly to your child care provider. This will change the way we currently make payments. You will no longer be responsible to pay for childcare using a two-party check. The last two-party payment that will be issued to both parents and providers will be made at the beginning of March for child care services for March. In the first week of April, you will receive a notice asking you to report any changes that will affect the April payment. You should report any changes as soon as you know of them. Changes can be reported up until the last working day in April. You as a parent will still be responsible to choose who you want to care for your child. You will still be responsible to report changes in your child care arrangements to your worker. Health and Safety Requirements for All ICCP Providers Also starting in April 2002, relative care providers must meet all health and safety requirements. If your child's aunt, uncle, grandma, grandpa, brother or sister takes care of your child and receives ICCP payments, they will need to meet this requirement. Your relative care provider will get more information on this change in the mail this month.
