The joints and ligaments that normally permit the spine to move become inflamed and stiff. The bones of the spine may grow together, causing the spine to become rigid and inflexible. Other joints such as the hips, shoulders, knees, or ankles also may become involved. Symptoms of ankylosing spondylitis appear most frequently in young men between the ages of 16 and 35. It is less common in women, whose symptoms are often milder and more difficult to diagnose. About 5% of ankylosing spondylitis cases begin in childhood; boys are more likely to have it than girls. When children develop ankylosing spondylitis, it usually begins in the hips, knees, bottoms of the heels or big toes and may later progress to involve the spine.
The gene is present in 8% of white Americans and 2-3% of African Americans. About 1% of the adult population has this condition and, as expected, the disease is three times more common in whites than in African Americans.

Arthritis - including peripheral arthritis and spinal involvement, usually involving also the sacroiliac joints. 2. They are Seronegative for Rheumatoid factor. 3. They may be involved with other medical problems - including... Psoriasis. Inflammatory bowel disease- ulcerative colitis or crohns disease. Post infective or reactive -to urogenital or gastrointestinal infections. Overlaps of these disorders may be seen. A strong genetic association with Class 1 HLA B27 gene (on chromosome 6) is described. Ankylosing spondylitis may also be referred to as Strümpell-Marie (or Marie-Strümpell) disease, Bekhterev-Strümpell syndrome, and spondyloarthritis. It is a chronic, progressive autoimmune disease characterized by arthritis, inflammation, and eventual immobility of a number of joints. The disease usually involves the spine and surrounding spinal structures and usually begins between the ages of fifteen and thirty-five years and affects young white males three times as frequently as females. Symptoms of the disease are generally milder among females. This disorder is seen rarely in people of color. Conventional treatment involves the use of Prednisone and other corticosteroids. Often patients are unable to tolerate the toxic side effects of these drugs. The alternative, nutritional medicine approach is to identify hidden allergies by blood test and avoid those allergens (primarily foods). Also enzyme potentiated desensitization holds hope for putting the disease in long term remission.

Ankylosing spondylitis is a condition which can lead to pain and stiffening in the spine and also to inflammation in joints in the arms and legs. Recognising ankylosing spondylitis often takes time because it may be disregarded either by the person affected or their doctor as simple backache. However, with a detailed story of the person's symptoms and an x-ray of the sacroiliac joints a rheumatologist should be able to make a clear diagnosis. Blood tests are not very useful. The information given below is intended chiefly for people who have been told they have ankylosing spndylitis by their doctor and may want to know more about its cause and treatment.
What sort of a condition is ankylosing spondylitis?
Although ankylosing spondylitis was at one time thought to be a variant of rheumatoid arthritis it is now clear that the two conditions have little or nothing in common. Ankylosing spondylitis is not an autoimmune disease of the sort in which there are antibodies to our own tissues. It seems more likely that a "volume control" on the way certain cells of the immune system generate inflammation is set too high. It could be seen as due to an overenthusiastic security system within the body. This may have advantages. It may provide better than average protection against "burglars", which for the body means infections. However, it also means that alarms go off in the middle of the night for no good reason. The exact mechanism is not clear, but probably will be by 2004.

Ankylosing Spondylitis (AS) Background AS is an autoimmune disease which affects collagen structures, specifically the joints between the vertebrae of the spine. In most cases the cause remains unknown. The commonality of symptoms and cause makes for a close association between AS and rheumatoid arthritis (RA).
[The American Medical Association Encyclopedia of Medicine, 1989, p. 111-112] More than 400,000 Americans suffer from AS, and it is most commonly seen in young men between 16 and 35 years of age, and afflicts approximately 1 in 1,000 people under the age of 40. Three times as many males as females are diagnosed with the disease, but this may be because females tend to have much milder symptoms and may never be diagnosed. It also appears in children (mostly boys), who account for roughly 5 percent of the cases. It is rarely seen in African-Americans. A genetic marker or tissue type (HLA-B27) is found much more often in people with AS than in the rest of the population; but 80% of the individuals with this genetic marker never develop the disease. It's not enough to have the genetic tendency; usually there is some event that somehow "switches on" AS, currently studies are looking into the possibility that a certain type of infection triggers AS onset.

However, if the cells in your body have similar surface membranes, your own immunity can be fooled and think that you are the invading germ and attack and kill your own cells.

What is juvenile ankylosing spondylitis?
Juvenile ankylosing spondylitis (JAS) is a type of arthritis that affects the spine and the sites where the muscles, tendons, and ligaments are attached to bone. "Ankylosing" means stiff or rigid, "spondyl" means spine, and "itis" refers to inflammation. The disease causes inflammation of the spine and large joints, resulting in stiffness and pain. The disease may result in erosion at the joint between the spine and the hip bone (the sacroiliac joint), and the formation of bony bridges between vertebrae in the spine, fusing those bones. In addition, bones in the chest may fuse. JAS is considered to be a multifactorial condition. Multifactorial inheritance means that "many factors" are involved in causing a health problem. The factors are usually both genetic and environmental, where a combination of genes from both parents, in addition to unknown environmental factors, produce the trait or condition. Often one gender (either males or females) is affected more frequently than the other in multifactorial traits. Multifactorial traits do recur in families because they are partly caused by genes. A group of genes on chromosome 6 code for HLA antigens play a major role in susceptibility and resistance to disease. Specific HLA antigens influence the development of many common disorders. Some of these disorders, such as JAS, are autoimmune related and inherited in a multifactorial manner. When a child has a specific HLA antigen type associated with the disease, he/she is thought to have an increased chance to develop the disorder. The HLA antigen associated with JAS is called B27. Children with the B27 HLA antigen are thought to have an increased chance (or "genetic susceptibility") to develop JAS; however, it is important to understand that a child without this antigen may also develop JAS. This means HLA antigen testing is not diagnostic or accurate for prediction of the condition. Males are affected with JAS three times more often than females.

DEFINITION: Ankylosing spondylitis is an autoimmune disease that causes inflamation of the spine and sacroiliac joints and can cause inflammation of the eyes, lungs, and heart valves. It varies from intermittent episodes of back pain that occur throughout life to a severe chronic disease that attacks the spine, peripheral joints and other body organs, resulting in severe joint and back stiffness, loss of motion and deformity as life progresses. A dramatic loss of flexibility in the lumbar spine is an early sign of AS. Although most symptoms begin in the lumbar and sacroiliac areas, they may involve the neck and upper back as well. Arthritis may also occur in the shoulder, hips and feet. As with many of the autoimmune diseases, many body systems can be effected in extreme cases.
AT RISK: Ankylosing spondylitis typically starts as an insidious onset of back pain in the late teens or early twenties, and seems to effect men more than women. I was diagnosed with Hep C about 2 years ago, and genotyped 1b. Because I have juvenile rheumatoid arthritis and ankylosing spondylitis my doctor recommended I do not have Interferon treatment. Are there any other options at this point or do I just wait until another treatment becomes available? Are there any other drugs in clinical studies at this time? My ALT and AST's are in the 70's. I appreciate your question and I also imagine you are a little frustrated. I understand that you have hepatitis C and you are not an ideal candidate for interferon based therapies because of your juvenile rheumatoid arthritis and ankylosing spondylitis.
The first question, I have for you is: do you know what your liver biopsy shows. If you have a benign liver biopsy with little scarring or fibrosis, you might not want to do anything right now. Hepatitis C typically causes liver disease over decades and doesn’t even cause cirrhosis in half of infected individuals. Typically women who do not drink alcohol are likely to have relatively slow progressing disease.

No. 5 Arthritis and Autoimmune Comorbidity The topic of arthritis can be dizzyingly complex, with not only many different types of arthritis, but different treatment strategies based on different theories of the pathogenesis of the various types. Arthritis can generally be classified as degenerative, metabolic, or immune-mediated, but the definitive separation among these classifications is often blurred as the pathogenesis is explored in greater detail. Spondyloarthropathy Now we get really complicated! We are in fact criss-crossing "families". Spondyloarthropathy is defined as a group of rheumatic diseases that share similar clinical features of axial arthritis, peripheral inflammatory arthritis and extra-articular symptoms such as conjunctivitis, skin rashes (eg., psoriasis), inflammation of mucosal surfaces and inflammation in the tendinous attachments to bone (enthesitis). Fine! sounds complicated but OK lets go along with that but then we learn that the family includes: Psoriatic arthritis - Which we have already covered as a separate section Ankylosing spondylitis - Which we have covered as one of the Psoriatic arthritis conditions Reiter’s syndrome and Enteropathic arthritis neither of which we have come across before! So here goes:
  • Reiters Syndrome
    Is a combination of arthritis, conjunctivitis and urethritis (or dysentery), which is called a "reactive arthritis" because joint inflammation that occurs in Reiter’s appears to be a reaction to an infection that originates in area’s of the body other than the joints. The most common pathogens known to induce Reiter’s syndrome are Shigella, Salmonella, Yersinia, Campylobacter or urogenital/post venereal pathogens such as Chlamydia or Ureaplasma.
  • Enteropathic Spondylitis
    Two inflammatory disorders affecting the intestines are known as Crohn's disease and ulcerative colitis, when peripheral arthritis and spondylitis occur in patients with Inflammatory Bowel Disease it is then considered as Enteropathic Arthritis
