ACCIDENT PREVENTION RECOMMENDATIONS COMPLEMENT RECENT GLOBAL AVIATION SAFETY INITIATIVES
MONTREAL, 1 October 1999 - Some 252 world aviation experts meeting at the headquarters of the International Civil Aviation Organization (ICAO), from 14 to 24 September, agreed on a series of recommendations designed to strengthen aircraft accident prevention through enhanced reporting systems and more efficient sharing of safety-related information. Participants at the 1999 Accident Investigation and Prevention (AIG) divisional meeting of ICAO represented 84 ICAO Contracting States of the Organization and 11 international aviation organizations. They focussed on modernizing Annex 13 to the Convention of International Civil Aviation – Aircraft Accident and Investigation – which establishes the standards and recommended practices (SARPs) for aircraft accident investigation and prevention. In his opening remarks, Dr. Assad Kotaite, President of the Council of ICAO, stated that “fundamental to prevention in aviation safety is the thoroughness of accident and incident investigations, and the timely reporting of the findings.” “Our investigation policies, procedures and techniques must keep pace with developments in aviation technology, worldwide expansion of air services, increased competition among airlines, liberalization of access to markets and the continuing trend of partnerships among airlines and manufacturers,” he stressed.

    Military Accident Investigation Reports
    When a military aircraft crashes, there is always at least one accident investigation; if it resulted in death or injury, there is generally more than one investigation, each conducted by the military. At times, a third may be conducted by the NTSB and/or the FAA. It is the usual case that two independent investigations are considered. The first is a formal Aircraft Accident Review Board, the composition of which is determined by regulations of the service involved. Since its purpose is limited to the promotion of aviation safety and often because the equipment and aircraft involved are complex and of a confidential nature, the military customarily refuses to make any disclosure regarding the composition of the Board, the factual findings made, or any report of its findings, opinions and conclusions. A second informal Collateral Board investigation is also prepared. In the Navy, this is made pursuant to the Judge Advocate General Manual and is referred to as the "JAG Report".
