I have specialty training from the University of Southern California, school of engineering in accident investigation. I have conducted numerous investigations and reconstructions for both plaintiff and defense law firms. I have deposition and trial experience in both state and federal courts. I consult about general aviation, wairbirds, experimental aircraft and some areas of commercial aviation. I am able to offer analysis and testimony in the areas of aircraft construction, flight operations, wreckage reconstruction, power plants, flight path reconstruction, aviation metallurgy, etc. My background in skiing and winter sports has allowed me to qualify in court and offer testimony in the areas of skiing technique, equipment failures, and skier responsibility. There is often a need for expert analysis of aircraft mishap causation beyond the official government report.
The National Transportation Safety Board (NTSB) is responsible for investigating aircraft accidents and incidents in the U.S., and foreign governments are responsible for investigating aircraft accidents and incidents which take place within their respective countries. However, governmental investigation procedures depend primarily on the type of aircraft involved. The NTSB rarely commits the resources for a full investigation i n cases of smaller aircraft accidents. The Office of Accident Investigation (AAI) is the principal organization within the FAA with respect to aircraft accident investigation and all activities related to the National Transportation Safety Board (NTSB). As such, AAI's mission is to investigate aviation accidents and incidents to detect unsafe conditions and trends and to coordinate the corrective action process. AAI accomplishes its responsibilities in a variety of ways including:

According to Annex 13 of the Civil Aviation Convention, paragraph 3.1, the purpose of aircraft accident and incident investigation is the prevention of accidents. This basic rule is also contained in the Investigation of Accidents Act (373/85) and European Union Directive 94/56/EC. The Air Accident Investigation Unit (AAIU) is part of the Department of Transport, and is responsible for the investigation of aircraft accidents and serious incidents within Ireland.
The Chief Inspector of Accidents reports directly to the Minister for Transport
The AAIU headquarters is located in Dublin.
The Air Accident Investigation Unit conducts its investigations of aviation accidents and incidents in accordance with Annex 13 to the International Civil Aviation Organisation Convention, European Union Council Directive 94/56/EC, and Statutory Instrument No. 205 of 1997. BACKGROUND INFORMATION ON THE AIR ACCIDENT INVESTIGATION UNIT Air Accident Investigation in Ireland was historically performed by the aeronautical staff of what is now the Department of Public Enterprise. At various stages in the past, this Department was known as the Department of Industry and Commerce, Department of Transport and Power, Department of Industry and Energy, Department of Energy and the Department of Transport, Energy and Communications, Department of Public Enterprise and currently the Department of Transport.
In 1993, the Irish Aviation Authority (IAA) was set up, as a self funding semi-state organisation, to regulate civil aviation in Ireland. The aeronautical staff of the Department went to the IAA, with the exception of one person. This person was retained to perform the Air Accident Investigation function within the Department. This was in keeping with a European Union Council Directive 94/56/EC which established the fundamental principles governing the investigation of civil aviation accidents and incidents of 21 November 1994, which came into effect on 21 November 1996. Article 6 of this directive states -
"Each Member State shall ensure that technical investigations are conducted or supervised by a permanent civil aviation body or entity. The body or entity concerned shall be functionally independent in particular of the national aviation authorities responsible for airworthiness, certification, flight operation, maintenance, licensing, air traffic control or airport operation and, in general, of any other party whose interests could conflict with the task entrusted to the investigation body of entity".

New NTSB Safety Recommendations
The Office of Accident Investigation is responsible for coordinating and tracking FAA responses to safety recommendations issued by the National Transportation Safety Board. The FAA must provide the Board with its course of action to address each initial safety recommendation within 90 days after the issue date. Following are the most recently issued safety recommendations for which the FAA is preparing an initial response.
