Sexual avoidance disorders are the primary complaint of many patients seeking help for sexual dificulties and have been recommended as a new diagnostic entity in DSM-III-R under the category Disorders of Sexual Desire.

Now, Helen Singer Kaplan, author of the widely used volumes, The New Sex Therapy and Disorders of Sexual Desire has written a pioneering book that provides the first integration of brief psychodynamic therapy with antipanic medication. She demonstraties that both psychotherapy and medication have valuable but entirely distinct functions in the treatment of these sexual disorders and both are needed to help patients with drug-responsive sexual anxiety.

Helen Singer Kaplan does an excellent job of separating out the psychological from the physical elements that contribute to having reduced sexual desire.Beginning with a distinction between genital dysfunctions and the arousal disorders, Dr. Kaplan proceeds through the importance of getting a complete and accurate picture of the patient's past and present experience that contribute to reduced or absent desire.After evaluation and a full understanding of the patient's goals for treatment, Dr. Kaplan moves into treatment strategies, using homework and other behavioral suggestions.She uses the exploration of underlying psychoanalytic causes only when the shorter-term solution-focused, behavioral treatments fail to make significant changes.The very readable case histories are a cross section of problems in this area and of Dr. Kaplan's innovative and individualized techniques for offering help.Her comfort with all areas of sexual behavior, anatomy, and individual preferences comes through in every sentence.

I was especially impressed by Dr. Kaplan's consideration of some people's lack of motivation in having insights into their personality patterns and behavior.She demonstrates admirable respect for people whose religious beliefs or ethnic norms prevent them from following some of her suggestions to fantasize, share fantasies, masturbate, or use erotica.In each case, she reframed the suggestion so that it was in keeping with the patient's values or she changed the assignment so that it was acceptable to the patient.Her understanding and respect that many couples have successful relationships that they want to maintain but that we, as therapists, might not want for ourselves, provided excellent role modeling for those who read her book.She also doesn't claim 100% success and is willing to discuss some of her treatment failures and why she thought she was ineffective-the mark of a true scientist.

Treating Sexual Disorders is an excellent, multi-authored work.It describes sex and couples therapy in a comprehensive manner that will be especially useful to the beginning sex therapist.I have one major concern, however, and that is the last chapter (treating paraphilia).The psychodynamic orientation of the author makes him regard paraphilia as inherently pathological, and not just as a result of learning history that gives us all different "turn ons."Charlton speaks, for example, on page 309 of how "working closely with the paphiliacs involves a confrontation with the nature of evil" and later mentions the "evil inherent in perverse sexuality."To be fair, perhaps he was only referring to victimizing or to the DSM IV caveat that the behavior has to cause upset to the person engaging in it or to a victim.The tone of the entire chapter, however, seems to pathologize paraphilia more than the literature supports.In my own practice (I am a licensed psychologist), I see many clients who have been told that any form of unusual sexuality makes them evil or dirty.They don't need any more help to feel that way.More could have been written regarding mutually consensual play between couples that is just a little (or a lot) "kinky" without the heavy judgement.This chapter aside, there is a great deal of value in this volume.

As a therapist who sees patients with sexual problems I found this book extremely helpful.The various chapters gave me the tools I needed to understand sexual difficulties from a psychological and physical perspective.I've even referred my patients to selected chapters of the book, and they have reported that they were able to understand the concepts without having a professional education.I would recommend this book to seasoned and novice therapists to add to their library.It addresses areas that therapists tend to avoid. ... Gina Ogden did an outstanding job of writing a wonderful contribution to understanding women's sexuality, specifically, low sexual desire. She brings years of experience as a therapist to her work as an academic researcher. The combination of using these two distinct roles to examine women's sexuality and sexual desire provides the reader with a very comprehensive and dynamic picture of what women's sexual desire is and how it can be enhanced. As a mental health counselor myself, I found that the way in which Gina describes women's sexuality in her book is a very important, and not regularly found, perspective in the mental health literature. Gina is able to write about low sexual desire from a clinical perspective, but also go beyond that to address the spiritual aspects of women's sexuality. I have found very few academic authors that delve into this realm with such tenacity and such a stand for women to be completely fulfilled in their minds, bodies and spirits.

The book encourages the reader to look at themselves as the source of their sexual desire, and strives to foster women's relationships with themselves as the starting point for a fulfilling sexual life. This is very important and empowering for women, who are many times taught by their caregivers and society to focus on factors outside themselves to fulfill themselves. Gina's book gives women permission to ask for what they want in sexual relationships and feel good about getting their needs fulfilled. In this book, Gina does a wonderful job of writing about the different aspects of why women's sexual desire may wane, which in my opinion could help readers find a context for their own experience. The book also provides the reader with some excellent exercises for getting back in touch with their own feelings of sexual desire, including a guided visualization and other exercises.

As a licensed mental health counselor specializing in sex therapy, I highly recommend this book to women who are struggling with concerns related to low sexual desire!

Return of Desire is a gift all women deserve.In this book Gina Ogden offers an understanding of the complexity of who we are as sexual beings and the connection between sexuality and spirituality.Regardless of one's religious or spiritual background (or lack of one), Gina helps us to realize how scripts that play out in our minds about our sexual selves may be influenced by family, culture and sources outside of us.She educates through insightful concrete examples and guided invitations to self reflect. I love this book "Return of Desire". I feel very peaceful when I am reading it... like talking with a friend! So many books make you feel like you are supposed to feel things that you just don't. Gina Ogden gives us permission to feel just exactly what we are feeling, but then helps us to explore "why" we might be feeling this way, and "how" we might be able to expand or incorporate new ways of thinking and feeling into our lives that ultimately will help us to be more whole. The structure of exploration into ourselves is very respectful and wholistic- I believe that we are creatures made up of physical, mental, emotional and spiritual components, and that all of these need to be taken into consideration when we want to grow. If you're interested in the subject the bibliography alone is worth the purchase. Great illustrations and a nice discussion with good primary source quotes on both upper/middle class women and working class women and their relationship with medicine from the turn of the last century.

Classic feminist text on women and medicine. Provides important overview on women's health in the 19th and 20th centuries. Discusses class issues as the impact on health (especially working women), public health, the medical profession, immigrants, prostitution, reformers like Margaret Sanger, etc. Still stands as essential reading for history of medicine.

I'm fairly stunned at the two two-star reviews here. This book was co-authored by Barbara Ehrenreich who went on to write several other WONDERFUL books, such as "Nickel and Dimed" and "For Her Own Good."

"Complaints and Disorders; The Sexual Politics of Sickness" is a short book (95 pages) but that's part of what made it such a good read. Once you start reading it, you won't want to stop until you're to the last page. And there's a lot of info packed into those pages.

On page 37, the authors write, "The entire mystique of female sickness - the house calls, the tonics and medicines, the heatlh spas - served, above all, to keep a great many women busy at the task of doing nothing."

That's what was done to women in the Victorian era. In modern times, we use beauty and weight as the lure to "keep women busy as the task of doing nothing."

There's also some background info on how women healers (or witches, as the men liked to call them) were removed from power so male doctors could enjoy their ascension to power and wealth.

It's one of a handful of books I've pressed into the hands of my daughter and said, "you have GOT to read this book." And *that's* the highest recommendation.

This book is way too short for such an interesting topic. Another problem is that it depicts women as main victims of weird medical practices in 19th and early 20th century, which is not entirely true. At those times (and may be true today in some cases also) one would be far better off if one stayed away from medical profession. Also, there are some really hillarious things about treating hysteria which authors didnot bother to mention. For the hystory of vibrators as legitimate medical treatment for hysteria, Rachel Maines has done very good job in dealing with this topic. What is also a pity is that authors didn't pursue various medical superstitions about females and different treatment of females which are present in modern times. For example, nobody questions the articles published in peer reviewed medical journal which state that patient's absolute refusal is "relative contraindication" for performing episiotomy. Is there any other surgical procedure which can be inflicted on a patient against patient's explicitly stated wishes? Or for example the fact that many health insurances cover Viagra but don't cover contraceptives. There are many topics well worth investigation which belog to the topic "Complaints and Disorders : The Sexual Politics of Sickness" which authors have chosen not to address in this pamphlet (can't even call it a book).

An account of modern (post-Industrial Revolution) medicine's attempts to squish women under the juggernaut of progress by declaring them weak and/or unfit to achieve. While we here at History House are always suspicious ofany work with an obvious political agenda (printed by The Feminist Press),the accounts of doctors and their loopy practices in the nineteenth centuryare a hoot. If push came to shove, we would suspect some of the wilderaccounts given are probably minority views, but the prevailing notion ofwomen as fragile bits of fluff subject to the whims of their productivesystems was probably held by a majority of the docs during the period inquestion.
