This is a major problem because obesity increases our susceptibility to many serious diseases, as most notably diabetes, but also cardiovascular disease, some forms of cancer and respiratory problems such as sleep apnea (stopping breathing while asleep). Today the health costs of obesity adds up to billions of pounds annually. Little wonder then that the WHO declared in 1997 that obesity is the largest health threat facing the western world. Changes in the prevalence of obesity over time are probably the result of changes in society rather than a result of genetics. The spread of car and TV ownership have altered our activity patterns, making us far more sedentary than previously, and we are now faced with a much wider range of foods from which to choose, many of which have high calorie and fat contents. However, faced with this so called 'toxic' environment we do not all become obese. Some people appear to have constitutions that are susceptible in the modern environment while others are resistant to the problem. These differences between people are mostly genetic.

Such was the case over 30 years ago in the field of endocrinology when investigators began probing the relationship between diabetes and obesity. Clinical studies showed that adult diabetics were often obese. What was the connection? Did obesity cause diabetes, or was it the other way around? In 1968, Dr. Daniel Porte, then an assistant professor of medicine at the UW, published an article in the American Journal of Clinical Nutrition in which he tackled these questions. Porte reported that although obese people were at risk for developing diabetes, their blood-sugar levels were not necessarily higher than normal. In other words, hyperglycemia, or high blood-sugar levels - the classic symptom of diabetes - appeared to be triggered by something else. This other trigger continues to baffle researchers and spawn original research. It is the mechanism for abnormal insulin secretion. Porte, now UW professor emeritus of medicine, is this year's Distinguished Scientist for the Science in Medicine Lecture Series. He will discuss his early work, as well as the latest developments in the field, in a lecture titled "Obesity and Diabetes Mellitus: The B-Cell Connection."

Extractions: On-line OMR's Axokine phase III analysis. April 8, 2003. On March 31, Regeneron Pharmaceuticals held a press conference with preliminary data on the phase III weight loss trial of Axokine, an analog of ciliary neurotrophic factor (CNTF). With the paucity of approved anti-obesity drugs, and the very modest results they provide, these results have been eagerly anticipated. The news for Axokine was very good at the end of phase II, with low levels of neutralizing antibodies, a very good safety profile, and an average 10 pound weight loss over 12 weeks. If this profile had held, Axokine was posed to be a "blockbuster" weight loss drug providing 20-30 pounds of weight loss for the average responder over the usual 6-9 month period during which weight loss drugs are at their most effective. Unfortunately the phase III results did not remotely mirror phase II. In our report we address: The weight loss effect, neutralizing antibodies and durability of Axokine.
